I investigated further and found that if the 360 extender is tuned to Live TV, when someone logs on or off Vista, Media Center loses the live TV signal. It can be restored by going to the Guide and selecting a different channel. This appears to be a problem only when the Fusion tuner is in use.

I'm sorry to say I can't remember exactly how I connected the Vista pc to the Xbox 360 the first time. I do remember that I didn't know there was an "add-an-extender" feature under tasks in the Media Center interface.
